Read the full article here 🤍 For more information including links to Noigroup courses head to 🤍 The Neurodynamic Techniques DVD and Handbook 🤍 🤍noigroup.com Here are some considerations: • A little bit often can be a safe place to start when the characteristics of the intervention are considered as well – slider or tensioner, passive or active, focussed or general. • Gentle tissue sliders may be more appropriate for nerves surrounded by acute inflammation • More robust mobilisation may be appropriate if there is scar tissue in or around nerve that is still remodelling. • Clinically reasoned trial and error is still permissible. • Test out the technique based on what you the clinician and the patient think is appropriate and practical and go from there. • A critical idea in modern approaches to treatment is the value of performing movement in a rich and diverse variety of contexts – indoors, outside, in the workplace, at home, at the in-laws house, in the shower, in the garden, feeling happy, feeling sad, in silence, with some music playing, in the location an injury occurred or in a safe-place – a rich variety of context is probably more important than a strict adherence to a regime.

Here is a shoe string model I designed for the visual learner (like myself) to better understand neurodynamics. Be sure to turn your sound on as I explain in more detail what is happening to the median nerve at different joints based on position. . Neurodynamics is the physical movement of nerves. Mobilization of nerves depends on the movement that is happening at the joints. However, it is important to note nerves are not exactly like shoe string, but perhaps more like dental floss. Nerves do not stretch like shoe string, but rather glide and elongate based on the balance of tension in the nerve beds at different joints. . According to Coppieters et al. 2009, "different types of nerve-gliding exercises have very different effects on the nervous system. A sliding technique is exclusively used to refer to the type of technique that involves simultaneous movement at 2 or more joints that have opposite effects on the nervous system." Demonstrated here at the end of the video is a slider technique exercise (with added wrist and finger extension as a progression) that has been shown to create the largest excursion amongst common median nerve slider exercises. This visual demonstration is purely a teaching model! Realize the shoe string model likely exaggerates the movement of the median nerve as the study found nerve excursion to be no more than 10.2mm! . McGill is a “distinguished professor emeritus”, University of Waterloo, where he was a professor for 30 years. He mentored over 37 graduate students during this scientific journey. During this time he taught thousands of clinicians and practitioners in professional development and continuing education courses around the world. MICHAEL SHACKLOCK DipPhysio, MAppSc, FACP NDS Instructor, founding director, principal ​Australia Michael received his graduate qualification at the Auckland School of Physiotherapy, New Zealand, in 1980. He worked in public hospitals in the medical, surgical, paediatrics, orthopaedics, geriatrics, intensive care and burns areas around New Zealand before being employed in private practices, treating spinal and sports injuries in Nelson with Michael Monaghan and Auckland with Margaret Franklin. In 1985 he moved to Adelaide, Australia, working at the Royal Adelaide Hospital and private practices around Adelaide. He completed a Graduate Diploma in Advanced Manipulative Therapy in 1989 and converted this to a Masters degree at the University of South Australia in 1993. He was awarded a Fellow of the Australian College of Physiotherapists (Australian Physiotherapy Association) for his monograph and international best-seller text book, Clinical Neurodynamics, Elsevier, Oxford, 2005. In addition to his neurodynamics book, he has published two other books, Moving in on Pain (Elsevier) and Biomechanics of the Nervous System: Breig Revisited. He also authored, co-authored and mentored numerous research studies, editorials, commentaries and clinical papers in international peer reviewed journals articles which have garnered many international awards, including Visiting Professor of the Faculty of Medicine, Barcelo University, Buenos Aires (see academic activities on ResearchGate). He was a member of the International Advisory Board of the journal Musculoskeletal Theory and Practice (formerly Manual Therapy), United Kingdom and is an Associate Editor of the Journal of Manual and Manipulative Therapy, USA. Here is a shoe-string model I designed for the visual learner (like myself) to help understand Neurotension. Be sure to turn your sound on as this time I explain movements at the hip, and what nerves are put on tension. As a clinician, you want to rule in and rule out what anatomy is involved in a patient's presentation. It's important to understand neurodynamics and positions of Neurotension. Neurodynamics is the physical movement of nerves. However, it is important to note nerves are not like shoe-string, but rather dental floss! Nerves do not stretch, they can slide, or get tensioned. Nerves need oxygen and blood flow just like the rest of our body. More importantly, the nervous system can be the primary limiting factor to movement. When looking at hip flexibility, it is essential to consider if the femoral, saphenous, or sciatic nerve are contributing to the clinical picture. Who is? MICHAEL SHACKLOCK DipPhysio, MAppSc, FACP NDS Instructor, founding director, principal ​Australia Michael received his graduate qualification at the Auckland School of Physiotherapy, New Zealand, in 1980. He worked in public hospitals in the medical, surgical, paediatrics, orthopaedics, geriatrics, intensive care and burns areas around New Zealand before being employed in private practices, treating spinal and sports injuries in Nelson with Michael Monaghan and Auckland with Margaret Franklin. In 1985 he moved to Adelaide, Australia, working at the Royal Adelaide Hospital and private practices around Adelaide. He completed a Graduate Diploma in Advanced Manipulative Therapy in 1989 and converted this to a Masters degree at the University of South Australia in 1993. He was awarded a Fellow of the Australian College of Physiotherapists (Australian Physiotherapy Association) for his monograph and international best-seller text book, Clinical Neurodynamics, Elsevier, Oxford, 2005. In addition to his neurodynamics book, he has published two other books, Moving in on Pain (Elsevier) and Biomechanics of the Nervous System: Breig Revisited. He also authored, co-authored and mentored numerous research studies, editorials, commentaries and clinical papers in international peer reviewed journals articles which have garnered many international awards, including Visiting Professor of the Faculty of Medicine, Barcelo University, Buenos Aires (see academic activities on ResearchGate). He was a member of the International Advisory Board of the journal Musculoskeletal Theory and Practice (formerly Manual Therapy), United Kingdom and is an Associate Editor of the Journal of Manual and Manipulative Therapy, USA. Michael presents keynote addresses and invited conference lectures, trains staff at elite sports medicine facilities and sports clubs, universities, hospitals and private seminars around the world. His special interests are neurodynamics, measurement of mechanical function of the nervous system, pain and the nervous system and musculoskeletal function. His most valued interest is how to synthesise science and clinical practice using evidence and reasoning. Michael is an active clinician, has 35 years of experience in treating patients and is recognised as an international leader in the diagnosis and treatment of musculoskeletal disorders with a neural aspect. 🤍

Also check out 🤍reactivept as they show a slider technique in a seated, weight-bearing position! What's a tensioner? Well watch the video and then imagine any athlete that uses their legs play their respected sport. Is the neurodynamic tensioner leg position necessary for some movements.. YES! Neurodynamic exercises can decrease abnormal mechanosensitivty, especially prior to an event. With increased mechanosensitivty in the posterior thigh if the hamstrings attempt to lengthen, there may be an increased stretch and/or pain sensation. Due to the abnormal sensation, the nervous system can slam on the movement system brakes by a protective muscle contraction. This movement limitation is interpreted as decreased hamstring length and functional range. If our nervous system slams the breaks while we are working hard, strains and tears can occur. So you need to TUNE your nervous system into the movements you need to perform. An article by Sharma et al 2016 {short term effectiveness of neural sliders...} concluded neural sliders and tensioners are both effective at immediately increasing hamstring flexibility.
